At The Jonathan Project there are two forms that the counselee(s) that are required and need to be filled out and/or signed.
The first is the Personal Data Inventory. This form is a great aid to the counselor in getting to know you, your history and the issue(s) that bring you to counseling.

The second form is titled “Understanding Biblical Counseling” which gives you the purpose and structure of the manner of counseling The Jonathan Project provides.
